Forms for the Doctor to Complete?
The following are some things you can do to help us fill out your forms.
Do You Have a Form for the Doctor to Complete?
Please Help Us By:
- Giving the form to the support staff before seeing your doctor
- Ensuring that your portion of the form is fully completed and signed (our staff can show you which portions are your responsibility)
- Having the contact information ready for where the form is going (full address including postal code, or fax number as appropriate)
- Understanding that
- There may be a charge for the form. Please discuss this with the support staff before seeing the doctor. Here is some information about charges for some of the more common forms: click here
- Our policy is to forward the completed form directly to the person or agency requesting it and to provide you with a copy if you desire it
- Being patient - many forms, especially longer ones, or ones needing a full review of your medical file, will take several weeks before they are ready!
