The pharmacist at Stonechurch Family Health Centre contributes her expertise about medications to the Clinic. She helps you and your prescriber make sure your medications are working for you. While the pharmacist does not fill prescriptions, she may be able to assist you if you have problems accessing your medications and she is a resource for anyone with questions about their medications. She is also available if you would like to learn more about managing your medications.

What pharmacist services are available?

You can meet with the pharmacist through an individual appointment or by participating in one on the clinic's groups.

During an appointment, the pharmacist will:

• read through your chart
• ask you questions about all your medications
• explain why you are taking each of your medications
• explain what side effects you need to watch for
• answer any questions you may have about your medications
• make suggestions to your doctor or other health care provider

Why would I meet with the pharmacist?

Meeting with the pharmacist does not necessarily mean there is a problem with your medications. The pharmacist will meet with you to make sure you are receiving the best medications and to help you understand the benefits and risks of your medications.

How do I make an appointment with the pharmacist?

You need a referral from your doctor or other health care provider for an appointment with the pharmacist. The reception staff will work with you to find an appointment time convenient for you and your doctor.

What should I bring along to my appointment with the pharmacist?

When you meet with the pharmacist, please bring along:

• all your medications, including all prescription and non-prescription drugs, vitamins, herbs and homeopathic medicines
• a list of your allergies
• a list of questions you have for the pharmacist
