Patient Resources

Both Stonechurch Family Health Center and McMaster Family Practice now offer multidisciplinary Diabetes Clinics. Evening and day hours are available. Patients will have the opportunity to meet with a physician and a dietitian, nurse practitioner or pharmacist. Foot care assessment is also available at the clinic. Patients are encouraged to bring a completed 3 day food diary along with their glucose logbook, medications and supplements. A variety of group classes are also offered.
